couple questions pcv and driveline

I have a couple questions for those who has dealt with either issue..

1. I did a couple launches this weekend to see how everything is going had no issue. this morning on way to work notices a slight vibration racked and checked car, rear cv (which I just rebuilt) loose cockeyed and one bolt broke.
I know they were torqued. has anyone else had issues with bolts backing out? I have new bolts on order.

2. I was driving around in manual mode and was in first gear in the neighbor hood. about 3000 rpm and started getting some pretty good smoke, don't have any under boost or idle. checked around and found oil in lines from valve covers to intake manifold and intake inlet pipe. so who is running a catch can and did you have this issue before or has it resolved it?  I read the sticky on this issue, I have a brand new pcv box, I cleaned the valve covers while it was all apart. I think I just need to ad a catch can/oil separator.
